Consent Form
A consent form is a document provided to individuals prior to their participation in a study. It clearly explains the purpose of the research, what participation involves, any potential risks or discomforts, the expected benefits, and the rights of the participants. The form also specifies how collected data or samples will be used and whom to contact with questions.
The form ensures that each person makes a free and informed decision with a clear understanding of the information provided. The form supplements discussions with the research team but does not replace them.
